   Yes, No, or Coach is the problem
   

3. Have you spoke to the District Representative?

   Yes, No, or District Rep. is the problem
   

4. Are you willing to write a formal complaint if requested?

   Yes, No, or Doesn't need to go that far,,, just FYI
